Training mishap, tech failure or human error: Tragic scenarios that may explain DC jet crash…& horrors passengers faced

INVESTIGATORS are desperately searching for answers around the doomed Washington DC aircraft crash that is feared to have killed 67.

An aviation expert told The Sun he believes several scenarios could be behind the deadly helicopter and plane collision – from a training mishap to a disastrous tech failure.

The official reason is still a “mystery” with all the black box data from both the passenger plane and the Black Hawk military helicopter yet to be recovered and decoded.

Aviation security expert Julian Bray says the crash “defies explanation” with a number of concerning and tragic possibilities running through his head.

He gave his initial predictions on what may have caused the fatal crash – citing several likely causes.

He told The Sun: “This should not have happened and we don’t know why [it happened].

“We’re looking at something that defies explanation at the moment.”

It was revealed after the collision that the packed plane was hit by a US Army Black Hawk helicopter as it was coming into land at Ronald Reagan National Airport.

Three soldiers were inside the chopper, which was on a training flight at the time, a US official confirmed.

Horror CCTV from the Kennedy Center showed the moment the aircraft burst into flames after being hit in the air – before plunging into the Potomac River.

Radio communications between the air traffic control tower and the Black Hawk show that the helicopter crew were aware that the twin-engine passenger plane was in the nearby vicinity.

But they failed to avoid it as both aircraft flew on the same flight path.

Two potential causes of the crash was mechanical failure on the part of the helicopter or simply human error.

“You’d expect the pilot of the helicopter to actually see the incoming aircraft, because not only would it be landing, but it had landing lights on,” Julian said.

“It’s also got repeater lights along the fuselage and is presumably lit up as well. You actually see the lights coming from the side windows, so it’s quite visible.

“They would know it’s there and that is the mystery because we can see the helicopter just going straight on its course and it doesn’t divert.

“It doesn’t descend. It doesn’t increase height. It just carries straight on.”


It comes as…


This lack of awareness of the surroundings points Julian towards believing the chopper might have been participating in an active training mission at the time.

He said: “The helicopter pilot might have been distracted or they might have been taking part in a training exercise where they deliberately turn off bits of kit and you have to do a workaround.”

It is common for trainee soldiers to take part in specialised aviation journeys with an experienced leader at the helm, Julian added.

However, answers are still yet to be uncovered on how the helicopter was allowed to be in the same zone as the plane that near to the runway.

Officials refused to comment on the same flight paths at a press briefing earlier today but confirmed it was “not unusual” to see in the DC area.

CRUCIAL BLACK BOXES

Rescuers are said to have recovered just a single black box from the jet so far with a number still missing in the wreckage and under the Potomac River.

Officials are hoping to find the others soon with vital information from the moments before the collision set to help piece together the crash.

Julian says until the boxes are all decoded then people just have speculation over the tragic cause of the smash.

He said: “What is really confusing is that I’ve seen some video of it, and very clearly the aircraft is just coming into land, so it can’t really do anything.

“It’s got right a way, if you like, because the helicopter is flexible, and should be able to divert and not crash into the aircraft.

“The military aircraft has also got all sorts of transponders and devices on board, not available on civil aviation, so they should have been able to see the incoming aircraft.

“We’ve got to wait for the black boxes.”

The aviation expert also noted that investigators will be searching for answers as quickly as possible in case a potential issue exists within other military aircraft.

RISING DEATH TOLL

The number of deaths is yet to be confirmed but 28 bodies have been pulled out of the water since last night.

A total of 64 people were on board the passenger plane, including four crew members, and three inside the military chopper.

Officials are now expecting the death toll to continue to rise throughout the day with no one believed to have survived the crash.

Washington’s emergency chief John Donnelly confirmed in a sombre press conference that the operations have gone from a rescue to recovery mission.

If all 67 passengers on both aircraft did die then it would make it the deadliest US air crash in nearly 24 years.

A “recovery centre” has even now been set up in the city to take in victims until they can be identified, reports local media.

A devastated Julian says based on his experiences and the harrowing crash footage the majority of passengers would have been killed instantaneously.

How common are domestic flights in the US?

EVERY single day the Federal Aviation Administration oversees 45,000 flights with almost three million passengers on board.

This covers a huge 29 million square miles of airspace from all over the US.

At peak times there can even be up to 5,400 aircraft in the sky at any moment – creating a mammoth task for aviation experts and air traffic controllers to maintain safety amongst all of them.

This means that any wrong call or misstep from either flight crew or controllers can result in a fatal tragedy.

A shortage of air traffic controllers in the past has caused delayed flights and raised a number of safety concerns.

Washington DC’s airspace in particular is extremely busy at peak times.

There have been several near-miss incidents at Reagan National Airport in recent years including a near-collision in May 2024 between an American Airlines jet and a small plane.

A similar catastrophe almost occurred a month earlier between a Southwest and JetBlue flight.

Reagan National also features a much shorter runway than many others with over 90 per cent of outgoing and in-going flights using the main one.

This makes it the busiest single runway in the US with over 800 takeoffs and landings each day – or one every minute for much of the day.

“It’s a terrible thing, but it’s a complete fireball, and you can see the debris exploding apart,” he added.

“When you combine the fuel on board the helicopter and fuel on board the aircraft then when the two collide that is quite an impact.

“So I don’t think there are any survivors from the initial collision and a lot of ambulances have now left the scene which is a bad sign.

“It’s moving from a search and rescue into a recovery operation and unfortunately we expect the death toll to be quite high.”

Julian also noted that the shallow river – which was only around 7.5ft in depth – would have acted as a brutal landing spot.

A sheet of ice was also believed to be sat on top which showed how bitterly cold the water was overnight.

This has led to fears that any miraculous survivors from the initial crash may have died from hypothermia.

One of the most shocking parts of the collision stemmed from the suddenness of it all for those on board the plane.

Passengers wouldn’t have known they were about to be hit due to the Americans Airline jet coming in to land meaning it was angled downwards.

Julian even predicted that the pilot and crew would have been unaware with their vision obscured.

He says this due to the distinct lack on panic on board.

No emergency information was reported with family and friends of the victims not sending any concerning texts either.

Julian believes those on board would have been simply sat in their seats with their seat belts on expecting to land.

Due to the landing process already underway he also feels the stewards would have done their routine safety checks and have sat down as well “waiting for a nice, gentle landing”.

Crash could have been prevented, Trump says

Posting on Truth Social, he said: “The airplane was on a perfect and routine line of approach to the airport.

“The helicopter was going straight at the airplane for an extended period of time.

“It is a CLEAR NIGHT, the lights on the plane were blazing, why didn’t the helicopter go up or down, or turn.

“Why didn’t the control tower tell the helicopter what to do instead of asking if they saw the plane.

“This is a bad situation that looks like it should have been prevented. NOT GOOD!!!”

Arne Slot’s men took top seeding position for the knock-out stage as Barcelona conceded a late equaliser against Atalanta, even though an under-strength Anfield side were beaten by PSV Eindhoven.

That secured their place as the top earners from the expanded competition so far, with the chance to add another significant sum of up to £43.9m in performance prize money in the knock-out stage.

Liverpool’s run saw them earn £15.61m for qualifying for the competition, plus a further £12.32m for their seven wins – with each victory worth £1.76m.

The Reds picked up a further £8.3m for topping the league phase table, £1.69m for finishing in the top eight and £9.22m for booking their place in the last 16.

In addition, Liverpool had begun the competition guaranteed £35.44m, from the new version of the TV and historical market “pillar”, worth £715.5m of the total £2.07bn prize fund, plus a £570,000 share of the leftover £10.55m from the 18 draws across the 144 matches.

Arsenal, who finished third in the table after their win at Girona, are the third highest earners so far.

The Gunners have picked up £75.9m from six wins and a draw in their eight games to also go straight through to the last 16. Their share of the TV and market pillar was £29.8m.

Barcelona, who split the two Prem sides in the table, also divided them in income so far, with the Catalans earning £76.3m.

Despite scraping through to the first knock-out stage with their win over Bruges, Manchester City are currently the seventh highest-earners in the competition so far.

They will be hoping to overcome Bayern Munich or Real Madrid in the first knock-out round.

So far, City have bagged £62.7m, mainly due to them taking the second biggest share – £37.3m – of the broadcast and past performance pillar.

That equates to just under £21m for every match they won in the group stage – against Club Brugge, Sparta Praha and Slovan Bratislava.

That leaves them behind the two possible last 16 opponents, with Munich having earned £66.8m and Madrid £66.5m so far, as well as PSG, who have banked £67.1m courtesy of the French TV deal being the biggest signed with Uefa.

But Pep Guardiola’s side would add a further £9.2m for winning next month’s last 16 clash, taking their total to £71.9m.

By contrast, despite the win over Celtic which confirmed their place in the top eight, Aston Villa have only banked £58.8m so far, due to the Villa Park side only taking a £15.6m share of the market pot.

‘Beautiful boys’ – Tributes for brothers who died within days of each other as mum shares heartbreak as 4th child dies

A HEARTBROKEN mum has been plunged into mourning again after receiving a phone call informing her of the death of her fourth son.

Mourners told how “no parent should have to go through that” as Christine Kavanagh opened up on the pain that comes with losing four sons and told how two of her children died this month.

Tributes have poured in for both Dillon, 30, and Damien, 36, from Gurranabraher in Cork city after their tragic deaths this month.

Dillon died last Thursday night of a suspected overdose whilst Damien was found dead in a flat in Cardiff, Wales on January 6.

Mum Christine said Damien had moved to Cardiff in 2019 to escape the drugs scene in his native city, two years before his brother Leon, 31, succumbed to drug addiction in 2021.

The brave mum’s story of sorrow began in 2016 when her son Ross, 31, collapsed and died of an epilepsy-related condition.

Speaking to Opinion Line Producer, Paul Byrne, on Cork’s 96FM, Christine praised her family and community for the support they have provided to her, particularly in the last fortnight as she has endured the unimaginable loss of two of her sons.

She said: “I would like to stop these drug dealers giving drugs and taking our children. These dirty drugs are after killing my son – my sons. Poor Ross died from epilepsy.

“If any dealer can hear my voice this morning please stop doing it. It just has to stop.”

The heartbroken mum added: “I don’t think I can take any more. Our hearts are crushed. Our world has ended.”

A notice on RIP remembered Dillon and Damien as the “dearly beloved” sons of Terry and Christine, cherished brothers of Dean, Aisling, Simone, Chloe, Nathan, Ryan, Alána and the late Ross and Leon.”

It said the Dillon would be sadly missed by his parents, family, aunts, uncles, brothers-in-law, sisters-in-law, nephews, nieces, relatives, kind neighbours and a “large circle of good friends”.

Tragic Damien was remembered as the “dearly beloved” dad of Deon and Kelvin, with the notice on RIP adding: “Sadly missed by his heartbroken parents, family, aunts, uncles, brothers-in-law, sisters-in-law, nephew, nieces, relatives, kind neighbours in Cork and Wales and a large circle of good friends, especially Michael Twomey.”

Both men will be laid to rest following a shared requiem mass in St Mary’s on the Hill Church at 10am on Saturday, with a funeral afterwards to St Catherine’s Cemetery in Kilcully.

‘MY HEART IS BROKEN FOR YOU ALL’

Damien and Dillon’s family have asked for donations, if desired, to go towards the Kevin Bell Trust – which aims to alleviate the financial hardship of bereaved families repatriating the bodies of loved ones.

Mourners have paid tribute to the tragic brothers, with one mum telling Christine and Terry that she “cannot begin to imagine the pain” that they are facing after losing their four sons.

She said: “My hope for you is that you feel their presence with you every day. No one person should have to endure so much sadness.

“I will keep you in my thoughts and prayers, and pray you find some inner peace, knowing their suffering is over is some small bit of solace for you I’m sure. Nothing can harm them anymore.”

Another mum added: “My heart is broken for you all. Sending love strength and courage to all who love Dillon and Damien.

“What handsome men they are. May they now rest in everlasting peace. I will remember you all in my prayers.”

Mourners told how Dillon was a “lovely boy”, describing him as “mannerly and always smiling”.

A tribute to Damien described him as a “great friend” with a “heart of gold”.

She took to TikTok to share her haul, as she said she just can’t resist buying things themed to her little girl’s moniker.

“My daughter’s name is Love, which makes shopping anywhere this time of year extremely damaging to our bank account because anything that has hearts or is pink or says Love is coming home with us!” she laughed.

She then took her daughter to her nearest Target store to see what romantic items they had in store.

“The good thing is I’m not getting a shopping cart,” she said.

“I’m only using the space we have under the stroller, so that should keep my shopping at a limit!”

She began with an ‘I (heart) you’ vase, as well as an adorable heart-shaped rug, some red and pink water bottle charms adorned with hearts, and some mugs with heart-shaped handles.

The mum then picked up some paint your own wooden hearts for her oldest daughter to do, before moving onto the clothing section.

Once there, she was immediately excited by what she found – a fluffy pair of cream PJs with red hearts on them.

“Oh my goodness. Lovey, we have to have this! You’re coming home with me!” she said.

There were even some heart-shaped water teethers that were perfect for her youngest, who is in the middle of teething, as well as a heart garland to go in baby Love’s room.

She showed the pile of bits in the base of the pram as she added: “I did not spend too much, I didn’t go too crazy, because I’m pretty sure we’re gonna be back again before Valentine’s Day!”

She captioned the video: “I’m scared for how much I would’ve got if I had used a cart!”

And in the comments section, lots of parents admitted they’d done the same with items themed around their kids’ names.

“My daughter’s name is Stormi so we get all cloud things!” one wrote.

“My daughter’s name is Story and she loves books – she has a huge book shelf in her room filled with them,” another added.

The footage snapped in Taizhou Zoo, Jiangsu Province, shows a small fluffy creature padding around a brick courtyard.

The animal is covered in black stripes and splodges over a carrot-orange background.

The fur ball is very obviously not a powerful adult tiger – but could be mistaken for a cub.

It then jumps up just below the camera, and we get a good view of its head.

From this angle, it’s clear the cuddly creature is not a tiger cub but a coloured canine.

The ears are too pointy and the orange looks unnatural.

Dogs dolled-up to look like other species has become quite an issue in China.

Taizhou Zoo has previously been caught parading painted puppies as pandas in order to pull in crowds.

The rubbish dye job was quickly sniffed out that time as well, and the zoo was forced to admit the animals were chow chows instead.

In September last year, another Chinese zoo was caught pulling the very same stunt.

Some zoo visitors even demanded their money back after noticing the “pandas” barking and panting.

Footage showed two odd looking “pandas” in an enclosure, with one furry animal sticking its tongue out towards the sun.

Clips taken by visitors appeared to show the pups with black ears, limbs, and dark circles around their eyes, but their canine features were still very much apparent.

The footage was shot at a park in Guangyuan, which allows visitors to see “rare and exotic animals”.

The zoo claimed it had not misled people as it didn’t declare the animals were pandas, explaining instead that they were, in reality, “panda dogs”.

Street scammers in Russia are also known to dupe tourists into paying for pics with pups they believe are pandas.

On one occasion, eyewitnesses claimed the “tiny [painted] panda cub” was being “tortured” as it was forced to be out in extreme 40C heat.

Shocked local residents in the village of Monastir, near the city of Sochi in south-western Russia, called the police when they spotted a street trader offering people the black and white bear.

A THUG left her victim with horror injuries after she slashed her in the neck with a broken bottle in a row over a taxi.

Sharnice Summer was within just 2mm of killing Bethany Lockyer when she attacked her outside a pub.

The 32-year-old had stolen a pre-booked taxi from the 28-year-old as she waited in the street with her family and friends in Poole, Dorset.

Summer refused to leave the cab – even when asked by the driver – before rowing with Bethany.

She left the taxi clutching a bottle in each hand and smashed one against the wall causing it to shatter.

The thug then slashed Bethany in the neck with the jagged weapon.

Summer, who has 13 previous convictions for 25 offences including violent attacks, admitted inflicting grievous bodily harm.

Despite her rap sheet, she was spared jail and instead handed a two-year sentence suspended for two years.

Speaking about the soft verdict, Bethany said: “The ambulance crew said if it had been two millimetres to the left I would have been dead. That was terrifying.

“She messaged me on the day of court case calling me a liar and said she had never stabbed me.

“I’m not happy about the sentence, she has been allowed to walk free while I was left for dead. It doesn’t feel like it’s enough.”

Bournemouth Crown Court was told the horror unfolded on March 1 last year following a night at the Churchill pub.

Charles Gabb, prosecuting, said: “The defendant, with a bottle of drink in each hand, started to antagonise members of the group. She was described as bolshy. She was told to go away and didn’t.

“She broke one of the bottles against the wall, selected one of the pieces of bottle and, using it as a very dangerous weapon, went for them as if to stab them.

“You can see just how close [the cut] must have been to the jugular. It was an 8cm long cut requiring stitches. It is not an exaggeration to say Miss Lockyer was extremely lucky.”

The court heard Bethany ran screaming into the pub as blood gushed from her neck before passing out.

Her sister-in-law saved her life by stemming the blood flow with tea towels.

Pictures show a huge wound across Bethany’s neck with blood splattered on her clothing.

In a victim impact statement, Bethany said the horror had triggered her existing PTSD and brought back old anxieties, which has caused her to avoid crowds and limit socialising.

She said: “I can’t cope with having to look at the scar every day. It is horrible and a constant reminder. Because it’s on my neck I am always being asked about what happened.”

Rachel Bailey, defending, said Summer’s evening had been “fraught with emotional issues” following a run-in with an ex and losing her mobile phone.

She said the thug “felt outnumbered” and had put her arms up to defend herself but did not “appreciate there was a glass shard in her hands”.

The lawyer added: “She does express sincere remorse. It was never intentional. Miss Lockyer had never deserved any of this.”

As well as a suspended sentence, Summer was placed on a tagged curfew between 7pm and 7am and ordered her to pay £1,500 compensation.

Judge Stephen Climie said: “The picking up of a piece of glass and waving it around in such circumstances was so dangerous, you could easily have killed somebody and you nearly did.

“The injury is an injury we commonly see coupled with a charge of murder or manslaughter.

“The appropriate punishment for street violence is almost always immediate custody. I am just persuaded there’s a sufficient prospect of rehabilitation to keep your liberty today, but if there is any breach of this order you will be serving the two years in prison.”

ALDI Ireland has released a game-changing air fryer – and it makes the perfect Ninja dupe.

Cooks can whip up tasty meals with 12 preset programs, a 60-minute digital timer, and variable temperature control.

The handy kitchen gadget also has overheat protection and a digital touch screen.

Shoppers must act fast, as this must-have kitchen item is set to sell out quickly.

Other kitchen items arriving in stores next week include a kettle, toaster, megastone pan sets, mini saucepans, and Stanley dupe bottles.

The Ambiano Textured Kettle and Toaster, just €14.99 each, are also arriving in stores.

The eye-catching kettle, with 1.7L capacity, comes in blue, white or yellow.

Pair it with the matching toaster, also available in the same colour range.

Included in the Ambiano range is a Milk Frother just €24.99.

“Suitable for all hob types including induction. Choose from 28cm Frying Pan or 28cm Griddle Pan.”

The stainless steel bottle makes a perfect Stanley dupe and is a bargain at €7.99.

They come in white, purple, pink or grey.

Max George in shocking ‘secret feud’ with bandmate Tom Parker’s widow Kelsey three years after his tragic death

MAX George is locked in a secret feud with bandmate Tom’s estranged wife Kelsey Parker, according to reports.

Tom died in 2022, at the age of 33, following a brave battle with brain cancer, leaving behind children Aurelia, five, and Bodhi, four.

Devastated Max vowed to take care of his family but insiders claim he has kept his distance from Kelsey and her young family in recent times putting a strain on their once good relationship.

A source told the Mail: “When Tom died, Max was quick to tell everyone how he planned to always be there for Tom’s wife and children, that he would take care of them and look after them.

“There was no reason to think he didn’t mean it and Kelsey was so grateful. But recently he has disappeared. He doesn’t visit her and the kids and isn’t involved in their lives.

“Actions speak louder than words. To put it quite simply, there’s no love lost between them at all anymore. Kelsey is very hurt that he’s cut himself off. Things are strained.”

Life has been tough for Max recently. He had a terrifying health ordeal of his own to contend with just before Christmas.

He was admitted to hospital with a heart condition that required emergency surgery and a pacemaker to be fitted.

The singer and actor shared photos from his hospital bed before the procedure showing his beats per inute had fallen to just 35.

Meanwhile, Kelsey recently announced she is expecting her first child with new boyfriend Will Lindsay, and teased they could wed in the next couple of years.

Attending a bridal event this week, she said she could see herself in one of the dress designs in the next couple of years.

Last year Max commemorated the second anniversary of Tom’s death by sharing unseen footage of them having a sleepover.

He captioned the bittersweet snap: “2 years without you my brother. Still doesn’t feel real. Found these today.. our last sleepover. I’ll cherish it forever.”

The pair regularly shared a room during The Wanted‘s final tour in 2022.

Tom, who was in the last stages of his life, was too sick to perform with the group though did accompany them onstage.

Kelsey also paid tribute, writing on her account: “Can’t believe it’s been two years. Feels like only yesterday that you left us. But I feel like I’ve been on my own for a long time now and so much has changed since you were here.”

She said she knew he’d be proud of her and the kids – from the way the family home was looking to Aurelia enjoying school and Bodhi being obsessed with football.

She continued: “They say the second year is the hardest and everyone keeps telling me if I can survive that, I can survive anything, but I’m somehow feeling weaker than I have before.
